New BPC SurveyManager Tutorials

We have started adding worked tutorials for survey layouts to the riskwiki. The tutorials include code you can copy and paste into your SM client to create the look and behaviours we describe.

Let me know others that you want, and we will endeavour to add them first.

We have also expanded the rules scripting documentation and added a page showing the input controls (or at least a large number of them). Various other improvements to the BPC SurveyManager documentation have been made on other pages in the riskwiki.
